29 CFR 1910.178(e)(2): Fork trucks, where the load presents a hazard, were not equipped with vertical load backrest extensions: a) On or about September 28, 2011, the Mitsubishi powered industrial fork truck, moldel FQ225K that was utilized in the warehouse area to load and unload material and supplies from the storage rack system did not have a load backrest to protect and minimize the unsecured material from falling rearward.
