29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    a.  On September 14, 2011, at the above addressed jobsite, an employees walking underneith a ladder jack scaffold was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access.    a.  On or about September 14, 2011, at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold, ten (10') feet seven (7") inches above the ground, was not provided with an access ladder, thereby exposing the employee to falls.

On or about September 14, 2011, at the above addressed jobsite, an employee was exposed to a fall while conducting roofing operations from a ladder jack scaffold, ten (10') feet seven (7") inches above the ground.

29 CFR 1926.454(a):  The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, 29 CFR 1926.454 (a)(1) through (a)(5):    a.  On or about September 14, 2011, at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold was not trained to recognize the hazards associated with scaffolding.  The employee was thereby exposed to the hazards associated with falls.
