29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer, the OSHA 300 log did not have an entry of the number of total number of restricted or transferred days in the "job transfer or restriction" workdays column. Located at the KDK Upset Forging Company, Blue Island, Illinois: On or about March 29, 2008, October 23, 2008 and June 9, 2009, the employer did not record the correct number of job transfer or restrictions resulting from work related injures or illnesses on the OSHA 300 log for the calendar years 2008 and 2009. On or about March 29, 2008, cutter- An employee sustained a sprain left hand while walking through the plant. The injury resulted in 45 days of restricted work activity. The OSHA log did not have a check mark in column I and the entry of 45 days was not made in column L. On or about October 23, 2008, helper- An employee sustained a left eye injury while grinding. The injury resulted in restricted work activity for 4 days, the entry of 4 days was not made in column L. On or about June 9, 2009, helper- An employee received an eye injury while using a grinding machine. The injury resulted in 2 days of restricted work activity. The OSHA log did not have a check mark in column I and the entry of 2 days was not made in column L.

On or about March 29, 2008, cutter- a 301 or equivalent was not filled out due to a work-related injury or illness to an employee resulting in the general recording criteria.
