29 CFR 1910.22(a): "Housekeeping." The floor of every workroom shall be maintained in an clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained and false floors, platforms, mats, or other dry standing places should be provided where practicable. At the work site, employees walked through the spin/wash area to monitor the process and equipment as well as to access other areas of the plant. Water constantly splashed out of the washing/mixing vat during agitation and formed a large puddle on the floor. There was no drainage provided next to the machine or mat(s) available that allowed employees to walk on a dry surface.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. a. At the work site, employees worked in the bale buster area opening bales of recycled products for processing. The back of the platform where the bales were loaded was approximately seven feet above floor level and did not have a guardrail. b. At the work site, employees used ladders to access the spin wash/dry area platform that was approximately seven feet eight inches above floor level. The platform did not have a guardrail.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: safe application, usage and removal of the energy controls. The employer did not provide adequate training on lock out/tagout procedures on the (but not limited to) grinder for authorized employees. At the work site, an unauthorized employee was observed not following the proper lock out/tag out procedures for the grinder.
