(a) Building 500, Pharmacy After Hours Dispensing Room - On or about September 29, 2011, The employer placed an employee work station and trash container in the exit access restricting the access to approximately 18 inches.    (b) Building 500, IV Room - On or about September 29, 2011, The employer obstructed the exit access by placing large cabinets against the door leading from the room, the exit access was restricted to approximately 15 inches.    (c) Building 500, Phlebotomy, Room 224 - On or about October 11, 2011, The employer placed a filing desk and hazardous waste disposal bin in the exit access restricting the access to approximately 18 inches.    (d) Building 500, Uniform Room - On or about October 12, 2011, The employer placed a work table in the exit access to prevent other personnel from entering the room, in doing so, the exit access for employees working in the uniform room was blocked.
