29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about September 22, 2011, an employee was exposed to falling into the moving blades of the American Process System ribbon blender (SN: 5606) in Blending Room B and resulting in severe injuries and death while loading the blender through an opening that was not fully grated. (b) On or about September 22, 2011, employees were exposed to severe cuts and amputations to their fingers during emptying operations on the American Process System ribbon blender (SN: 5606) in Blending Room B and the discharge opening was not guarded. (c) On or about September 22, 2011, employees were exposed to severe bodily injury and death from access to the moving ribbon blade in the American Process System ribbon blender (SN:5606) in Blending Room B as the safety interlock devices on the excess side panel doors were bypassed. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
