29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: (a) Gonzalez Roofing at 4478 Hayler Ave., Loveland, CO 80534: On 9/26/11, the exposing employer did not ensure that employees eyes were protected while working in the vicinity of, and using pneumatic nail guns. Employees were not wearing safety glasses. Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety nets systems, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Gonzalez Roofing at 4478 Hayler Ave., Loveland, CO 80534: On 9/26/11, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in re-roofing activities while working at the unprotected edges of a roof on a single family residence. Employees were exposed to a fall hazard of approximately 20-25 feet.
