29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. In energized installations each outlet box did not have a cover, faceplate, or fixture canopy:      a) Main floor utility area; On or about 9/20/11 and at times prior thereto, cardboard was used as a temporary cover for an energized 240 volt/200 amp circuit breaker panel in lieu of the manufacturer's cover. This exposed Conrad Brothers of Idaho and their subcontractors' employees to energized parts.    b) Basement utility areas; On or about 9/20/11 and at times prior thereto, cardboard was used as a temporary cover for two energized 240 volt/100 amp circuit breaker panel in lieu of the manufacturer's cover. This exposed Conrad Brothers of Idaho and their subcontractors' employees to energized parts.    c) Various locations in the basement, main floor and upstairs; energized 120 volt/20 amp GFI duplex receptacles without faceplates were being utilized for providing power to various stationary and portable power tools. This exposed Conrad Brothers of Idaho and their subcontractors' employees to energized parts.    Note: Abatement certification is NOT required for these items.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.      a) The second floor stair landing/balcony; On or about 9/20/2011 and at times prior thereto, the Conrad Brothers of Idaho's employees and their subcontractors' employees were performing work on the second floor of a residential structure and were traveling along sections of the landing/balcony that were not fully guarded.    b)  West exterior deck; On or about 9/20/2011 and at times prior thereto, a subcontractor's employee was utilizing the deck to access one side of the structure to the other without a means of fall protection.    c)  East exterior deck, 2nd floor; On or about 9/20/2011 and at times prior thereto, a ladder was placed for accessing the 2nd floor from the exterior deck without a means of fall protection.      Note: Abatement certification is NOT required for these items.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:      a) Stairway between the main and second floors; On or about 9/20/11 and at times prior thereto, On or about 9/20/2011 and at times prior thereto, the Conrad Brothers of Idaho's employees and their subcontractors' employees were performing work on the second floor of a residential structure and were utilizing the stairs without the handrails/stairrails being installed.    b) Stairway between the basement and the main floor; On or about 9/20/11 and at times prior thereto, On or about 9/20/2011 and at times prior thereto, the Conrad Brothers of Idaho's employees and their subcontractors' employees were performing work in the basement of a residential structure and were utilizing the stairs without the handrails being installed.    Note: Abatement certification is NOT required for these items.
