29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. In energized installations each outlet box did not have a cover, faceplate, or fixture canopy: a) Main floor utility area; On or about 9/20/11 and at times prior thereto, cardboard was used as a temporary cover for an energized 240 volt/200 amp circuit breaker panel in lieu of the manufacturer's cover. This exposed other subcontractor's and their own employees to energized parts. b) Basement utility areas; On or about 9/20/11 and at times prior thereto, cardboard was used as a temporary cover for two energized 240 volt/100 amp circuit breaker panel in lieu of the manufacturer's cover. This exposed other subcontractor's and their own employees to energized parts. c) Various locations in the basement, main floor and upstairs; energized 120 volt/20 amp GFI duplex receptacles without faceplates were being utilized for providing power to various stationary and portable power tools. This exposed other subcontractor's and their own employees to energized parts. Note: Abatement certification is NOT required for these items.
