29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: a) Alvarados Roofing Inc. at 608 Sedgwick Drive, Fort Collins, CO 80525: On or before September 24, 2011, the employer did not ensure that employees were using fall protection when shingling a two story residential roof with a 6 in 12 pitch. Condition exposed employees to fall hazards of up to 20 feet. Abatement note: Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet"). Alvarados Roofing Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 314660010, citation number 1, item 1, issued October 22, 2010 and became a final order on Descember 19, 2010, with respect to a workplace located at 10711 Idalia Way, Commerce City, CO 80022.
