29 CFR 1926.52(d)(1):  In all cases where the sound levels exceeded the values shown in Table D-2 of 29 CFR 1926.52, a continuing, effective hearing conservation program was not administered:    The employer did not provide a continuing, effective hearing conservation program which included exposure monitoring, baseline audiograms, annual audiograms and annual training for employees who were exposed to noise levels such as:    (a) On October 5, 2011 an employee who was blasting a railroad bridge was exposed to approximately 7401% of the PEL or 125.5 dBA for the 272 minute sampling period, which included breaks and one stop to wait for a train to cross the bridge.    (b)  On October 5, 2011 an employee who was blasting a railroad bridge was exposed to approximately 490% of the PEL or 105.8 dBA for the 276 minute sampling period, which included a two hour period outside the containment area, breaks and one stop to wait for a train to cross the bridge.
