29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. (a) Lewandowski Brothers, Inc. on or about October 04, 2011, at the address of 1420 S. Holland-Sylvania Rd. Holland, OH: The employer failed to ensure an employee working from a Genie GS-1930 scissor lift was adequately protected from falls. The employee was not tied off to the platform and was observed standing on the top of the guardrail, effectively bypassing all fall protection measures. The employee working from this condition was exposed to an approximate 25ft fall to the ground below.

Holland-Sylvania Rd. Holland, OH: The employer failed to ensure an employee working from a Genie S-65 aerial lift was adequately protected from falls. The employee was not tied off to the basket of the stated aerial lift while it was in use. The employee was exposed to an approximate 30ft fall to the ground below.
