29 CFR 1910.213(h)(1):  The sides of the lower exposed portion of the blade of radial saw was not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the meterial.    a) In the Carpenter Shop the Dewalt radial arm saw Model GE sn9631 did not have a lower blade guard.

29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials, a minium distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire resistance rating of at least one-half hour.    (a) In the Welding Shop oxygen and acetylene bottles were stored together.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) The electrical panel in Gas Station was open and damaged allowing exposure to live electrical parts.    b) Florescent tube lights in warehouse next to storage shelves were not guarded.    c) Throughout warehouse screw in florescent lights did not have guards to protect them against breaking.

29 CFR 1910.305(g)(2)(iii):  Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    (a) In the Welding Shop the power cord for drill press was deamaged in that the outer insulation sleve had pulled loose from the plug allowing stress to be transmitted to the terminal screws should the cord be pulled.
