29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about September 20, 2011 and times prior, employees were exposed to injuries from falls greater than 6 feet while installing roofing materials. Fall protection components including rope grabs and shock absorbing lanyards were available but not used. Additionally employees connected their rope to the front D-ring of their harness.
