29 CFR 1910.178(p)(1):     Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)    (a)  Description:  The parking brake of the Hyster Powered Industrial Truck (PIT), Serial Number H177B16913X, was not operable thus, indicating a defective parking brake in need of repair.      Location:  Warehouse Ramp    Potential Outcome:  Failure to properly examine/inspect PIT to ensure the integrety of the parking brake system may potentially result in unsafe operating condition(s) thereby, potentially resulting in serious personal injury (struck-by), fatality, or property damage.         (b)  Description:  While utilizing the powered industrial truck (serial number, H177B16913X) , operator activated the horn by grasping the insulated wire and making contact with a metallic surface on the steering wheel thereby, indicating a defective horn, and in need of repair.     Location:  Warehouse Ramp    Potential Outcome:  May affect the operators ability to react in a timely fashion to warn others thereby, potentially exposing other employees to struck-by hazards, causing potential serious injury, fatality, and/or property damage.
