29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clearand in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked. On 10/19/2011, the forge shop southeast aisle and passageway were not permanently marked.

29 CFR 1910.218(a)(2)(iv): All overhead parts shall be fastened or protected in such a manner that they will not fly off or fall in event of failure. On 9/28/2011, at the forge shop, the 2003 hammer ram and die block were not adequately fastened in that the use of shrink fit was the only means of fastening.
