29 CFR 1926.503(a)(1):       29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards which enables each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize the hazards    Employees were exposed to fall hazards while performing roofing activities without appropriate fall protection in place and were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize the hazards.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and  the ladder side rails did not extend at least three (3) feet above the upper landing surface to which the ladder was used to gain access:    Employees installing shingles were exposed to a fall hazards of approximately 18 feet to a lower level, due to the fact that the ladder used to access the roofs did not extend three (3) feet above the upper landing surface.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    Employees were exposed to a falls of approximately 14 feet to 18 feet when using extension ladders to access the roof to perform roofing operations and the side rails of the ladder extended approximately two (2) foot above the landing surface. The employer did not provide training for each employee that would have enabled them to recognize the hazards and provided them with the procedures to follow to minimize these hazards.
