29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: (a) In the lathe department, the interlocking mechanism on the Mori Seiki #812 CNC Lathe was removed from the door, allowing the machine to operate with the door open, exposing employees to entanglement hazards. (b) In the lathe department, the Okuma LB-8 #806 CNC Lathe was operating with the interlock keyed in the off position, exposing employees to entanglement hazards. (c) In the lathe department, the Okuma LB-8 #806 CNC Lathe had an unguarded rotating projection 40 inches off the floor, exposing employees to entanglement hazards. (d) In the lathe department, the Okuma LC40 CNC Lathe did not have any interlocking or permanent guards, exposing employees to entanglement hazards.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) In the storage dock area, a RACO metallic utility box had a receptacle with a grounding pin broke off in it and another with a burn mark. The utility box was in service powering a floor fan. Employees were exposed to electrical hazards.
