29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. Employees conducting residential construction operations on a roof approximately 30 feet above grade were exposed to fall hazards. Fall protection was not utilized.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926: Employees using a stairway to access and egress upper levels were exposed to fall hazards of 14.5 feet. The stairway landing was not equipped with fall protection.
