29 CFR 1910.304(g)(5): The paths to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective. a) On 10/07/11 at the workplace, in the New Addition, a Pepsi Refrigeration unit, manufactured by True Refrigeration, was ungrounded causing the unit's casing to become electrically live. The ground had been bypassed using a two pronged current tap with the ground terminal unconnected. b) On 10/07/11 at the workplace, in the New Addition, a Hussman Cooler was ungrounded causing the unit's casing to become electrically live. The ground had been bypassed using a two pronged current tap with the ground terminal unconnected.

29 CFR 1910.305(g)(1)(iii): If used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from an approved receptacle outlet. a) On 10/07/11 at the workplace, in the New Addition,flexible cords were used to supply power to various equipment, including but not limited to, a Hussmann Cooler. These extension cords were daisy chained to a power strip.
