29 CFR 1910.266(d)(2)(i): The employer did not provide first-aid kits at each work site where trees were being cut, at each active landing, and/or on each employee transport vehicle:    a) Jobsite: On or about September 28, 2011 first aid kits were not at each work site where employees performed tree thinning activities which required the use of chainsaws to fell and buck trees.    b) Jobsite: On or about September 28, 2011 first aid kit(s) were not available and/or employees performing tree thinning activities which involved the use of chainsaws to buck and limb felled trees were not aware of the location of a first aid kit at their work site(s).       Note) Abatement certification IS required for this item.

29 CFR 1910.266(i)(10)(i): The employer did not verify compliance with paragraph (i) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature:    a) Jobsite: On October 7, 2011 and times prior thereto, the employer did not determine and certify if the first aid and CPR training employees may of had in the past, was adequate prior to allowing them to perform tree thinning activities.      Note) Abatement certification IS required for this item.
