29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Eldorado Construction at 4120 Julesberg Dr, Loveland, CO 80538: On and before October 1, 2011, the exposing employer did not ensure that employees were protected from injury, in that two employees installing shingles on a 5-in-12 pitch roof on a one-story residential home were not utilizing a fall protection system. This condition exposed employees to a fall hazard of approximately 9 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Eldorado Construction at 4120 Julesberg Dr, Loveland, CO 80538: On and before October 1, 2011, the exposing employer ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees worked on a single-story, 5-in-12 pitch roof, on a residential home without the use of a fall protection system. This condition exposed employees to a fall of approximately 9 feet.
