29 CFR 1926.52(d)(1): In all cases where the sound levels exceeded the values shown in Table D-2 of 29 CFR 1926.52, a continuing, effective hearing conservation program was not administered: An employee performing blasting operations, while wearing hearing protection, was exposed to continuos noise at 1822% of the permissible exposere level (8-hour time weighted average sound level of 90dBA) or an equivalent sound level of approximately 110.9dBA during the 336 minute sampling period on October 24th 2011; exposure calculations include zero increment for the 144 min not sampled and the employer did not institute an effective hearing conservation program.
