29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls areacquired by employees:      (a)  The training on energy control procedures provided to employees operating the coil line machine was inadequate, in that employees were not aware that they needed to isolate and then attach a lock to the energy isolating devices, prior to changing tensioner belts and rollers, prior to removing the shear guard and flattening out the metal, and prior to wiping down the pinch rollers.

29 CFR 1910.147(d)(3):  Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    (a)  Employees changed the tensioner belts and rollers, wiped down the pinch rollers, flattened out the metal, and pulled metal out that got caught in the shear, without first isolating the energy sources.

29 CFR 1910.147(d)(4)(i):  Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees:    (a)  Employees changed the tensioner belts and rollers, wiped down the pinch rollers, flattened out the metal, and pulled metal out that got caught in the shear, without first affixing a lockout device to the energy isolating devices.

29 CFR 1910.242(a):  Each employer shall be responsible for the safe condition of tools and equipment used by employees, including tools and equipment which may be furnished by employees:    (a)  On or about August 10, 2011, an employee performing yard work on company property, was exposed to being cut by a 10-inch radial saw blade that was attached to a Stihl Triimmer, in that the saw blade was not a manufacturer approved attachment, the blade did not have an adequate guard, and the Stihl Trimmer did not have the appropariate shoulder harness that was required by the manufacturer.

29 CFR 1910.132(d)(1):  The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:      (a)  The hazard assessment was inadequate, in that it did not include an assessment of employees performing work outside the building, including employees performing yard work using portable powered tools.
