29 CFR 1910.303(b)(1)(iv): Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety equipment shall be determined using the following considerations: (iv) Electrical insulation; a. Outbound Loading Dock/Eastside of the Building - On or about October 17, 2011, employees were exposed to electrical hazards due to equipment which was not free from recognized hazards. The electrical equipment was the following; 1. Between Loading Dock door # 113 and Loading Dock door # 114 - the insulation on the electrical cord for the electrical saw De Walt (Serial # 144766 & Model # DW713) was pulled away from its fitting. 2. By Loading Dock door # 122 - the insulation on the electrical cord for the electrical saw De Walt (Serial #189202 & Model # DW713) was pulled away from its fitting.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. Outbound Loading Dock/ Eastside of the Building - On or about October 17, 2011, employees were required to load and unload trucks where there were incandescent dock lights - (Phoenix Dock Lights - DL-INC-LH) fixed to the building's structure. The electrical cords for some of these lights were loose from their metal casing assembly. The loose flexible cords were found at the following locations; 1. The flexible cord for the incandescent dock light near the door with # 91. 2. The flexible cord for the incandescent dock light near the door with # 95. 3. The flexible cord for the incandescent dock light near the door with # 99. 4. The flexible cord for the incandescent dock light near the door with # 106. 5. The flexible cord for the incandescent dock light near the door with # 107. 6. The flexible cord for the incandescent dock light near the door with # 119. 7. The flexible cord for the incandescent dock light near the door with # 111. 8. The flexible cord for the incandescent dock light near the door with # 112.

29 CFR 1904.29(b)(1): Information shall be enter on the OSHA 300 Log, entering the description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of each year. a. Entire Facility - On or about October 17, 2011, the employer did not provide the required injury or illness descriptions on the OSHA 300 Log for the calendar year 2011. 1. On or about January 25, 2011, a recordable injury occurred when an employee's right thumb was caught between 2 parts while repacking material. 2. On or about March 9, 2011, a recordable injury occured when an employee's left knee struck a transmission rack when turning around to leave the area.
