29 CFR 1926.453(b)(2)(iv):  Employees were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    a)  Arrow Flooring and Painting LLC at 560 Silverthorne Lane, Silverthorne, CO 80498:  On or before October 13, 2011, the employer extended the reach of an elevated manlift by allowing employees to set a 20 foot Werner portable metal extension ladder, model D 1220-2, on the floor of an elevated Genie S-60 manlift to paint near the eave of a six story La Quinta motel.  Condition exposed employees to fall hazards of up to 60 feet.    Abatement note:  Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a)  Arrow Flooring and Painting LLC at 560 Silverthorne Lane, Silverthorne, CO 80498:  On or before October 13, 2011, the employer allowed employees to work in an elevated Genie S-60 manlift without connecting their lanyards.  The employees also moved the Genie manlift with the employees in the elevated platform without being tied off.  Condition exposed employees to fall hazards of up to 50 feet.     Abatement note:  Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a)  Arrow Flooring and Painting LLC at 560 Silverthorne Lane, Silverthorne, CO 80498:  On or before October 13, 2011, the employer did not train employees of the hazards associated with the use of the Genie S-60 manlift, including the nned to:    1)  tie off when working in an elevated manlift;    2)  lower the manlift when moving it;    3)  tie off when employees are in the manlift when moving it; and    4)  stand firmly on the floor of the manlift without extending the reach through the use of ladders.    Condition exposed employees to fall hazards of up to 60 feet.
