29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface. Employee was installing sheet metal roof at a height of 30 feet from the ground, using a wooden scaffold that was built without midrails.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Employees were using an 8-foot portable metal ladder to access the roof of the Methodist Church. The ladder did not extend at least 3 foot above the roofline.
