29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift;  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:    Employees were working from an International truck mounted L55 aerial lift platform at a height greater than twenty-five (25) feet above a pavement surface without utilizing personal fall arrest systems.      No Abatement Certification or Abatement Documentation required.

29 CFR 1926.503(b)(1):     The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:      Employer did not provide certification of employee fall protection safety training on employees who were working from an aerial lift platform at a height greater than twenty-five (25) feet above a pavement surface without utilizing personal fall arrest systems.      Abatement Certification or Abatement Documentation required.
