29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: At the Mechanical Equipment Building on or about October 13, 2011, and at time prior thereto, glove were not used or butyl or nitrile gloves were not used while mixing and using mortar, containing Portland cement, to lay CMUs.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage: Near the south side of the Training Center on or about October 13, 2011, a 110 VAC extension cord was not protected from damage as it lay on the compacted soil with one tire of an JLG aerial lift parked on it. The cord powered 110 VAC drills used to drill holes in the precast concrete for doll placement. The cord appeared in good condition and was protected with a ground fault circuit interrupter.
