Gillette Roofing, LLC. at the location of 1113 Cardinal Bay Dr. Oregon, OH; On or about October 13, 2011, the employer did not assure that employees were protected from fall hazards while performing roofing activities on a low slope residential structure. Employees were exposed to a fall of up to 8ft to the grass below.
