29 CFR 1926.403(b)(1):     Examination. The employer shall ensure that electrical equipment is free from recognized hazardsthat are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the followingconsiderations:      1)     At the worksite, the employer did not ensure that the extension cord used to supply power to the portable grinder, not be damaged such as, but not limited to being cut down do the bare wire, thereby exposing employees to a potential shock hazard.

29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent andcontinuous.          1)     At the worksite, the emplooyer did not ensure that the extension cord end, that was used to supply power from the generator to the portable grinder, was equipped with a ground pin, thereby exposing employees to a potential shock hazard.

29 CFR 1926.1053(b)(16):     Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.          1)     At the worksite, the employer did not ensure that the ladder that was used to access/exit the trench box was not defective.
