29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. (a) At the entrance to the construction site, employees cutting and tying rebar were not using protective gloves to protect the hands against punctures and/or lacerations to the hand.

29 CFR 1926.404(e)(1)(i): Conductors rated 600 volts, nominal, or less did not have sufficient ampacity to carry the load: (a) At the rear (exterior) of the warehouse, a copper roundstock was used in lieu of a fuse in a 30 amp Square D safety switch thereby, preventing interruption of current at appropriate fault levels, potentially resulting in electrical shock and/or fires.
