29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : Mare Island VA Clinic - On or about August 8, 2011 and at times prior thereto, annual audiometric testing was not conducted on the Maintenance Engineer determined by VA Memorandum dated May 8, 2001 to require enrollment in the Hearing Conservation Program based on noise dosimetry conducted by the VA Industrial Hygienist. Audiometric testing was conducted biennially.
