29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided where practicable: (a) On or about October 6, 2011, employees were exposed to slip and fall hazards while washing dishes in the kitchen at Southridge Health Care Center, located at 3600 South Norton Avenue, Sioux Falls, South Dakota, and mats were not supplied to prevent slipping on floors where water had accumulated.

29 CFR 1910.1030(h)(5)(i): The employer did not establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps: (a) For calendar years 2008, 2009, and 2010, the employer (Southridge Health Care Center) at 3600 South Norton Avenue in Sioux Falls, South Dakota, recorded needlestick injuries on the OSHA Form 300 Log of Work-Related Injuries and Illnesses because a sharps injury log for recording percutaneous injuries from contaminated sharps had not been established or maintained.
