29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  (a) Welding - on Augsut 15, 2011 and at times prior thereto, the metal finishing/polishing task utilizing glass beads was not evaluated to determine whether any hazards were present/associated with the task necessitating the use of personal protective equipment to protect from eye and inhalation hazards.    (b) Painting - on August 15, 2011 and at times prior thereto, the powder coating operation utilizing paint as a spray finish was not evaluated to determine whether any hazards were present/associated with the task necessitating the use of Personal Protective equipment to protect the employee from skin and inhalation hazards.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)  AMSA 13 - on or about August 15, 2011 and at times prior, a written hazard communication program describing how the criteria in sections (f), (g) and (h) of the section would be met or the methods AMSA 13 would use to train employees on the hazardous tasks and associated chemicals for hazardous substances including, but not limited to, glass polishing beads and powder coatings capable of causing respiratory irritation at minimum.

29 CFR 1910.1200(g)(8):     The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:  a)  AMSA13 - on August 15, 2011 and at times prior thereto, material safety data sheets were not maintained in the workplace for powder coatings used for painting and glass beads used to finish/polish metal parts.

29 CFR 1904.29(b)(1):     What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  a)  Mare Island AMSA 13 - On or about August 15, 2011 and at times prior to, the California Army National Guard did not complete the 2008, 2009, 2010 and 2011 OSHA 300 form(s) columns E and F in the detail required by the standard and indicated on the form.  Column E would state, maintenance shop or maintenance shop bay, but not the specific facility or city/state where the maintenance shop was located.  Column F did not contain information on how the injuries were sustained and/or what the employee was doing when they sustained the injury or what injury was received in other instances as a result of the injury and finally in some instances did not indicate the exact location of the injury such as left or right .  Overall, the column did not provide information making it possible to understand what injury/illness occurred or evaluate the information to determine any trends.  The logs cover employees at facilities throughout California and other state(s) not just AMSA 13.
