29 CFR 1926.651(c)(2):     A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On October 17, 2011 employees were not provided a safe means of access/ egress to the trench they  were working in, exposing them to the hazards associated with no safe access.     Town & Country Underground, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 19263651(c)(2) which was contained in inspection number 314420308, citation number 01, item number 01a, and was affirmed as a final order on 08/04/10, with respect to a workplace located at Harrison St. & S. Frontage Rd. Hillside, IL.    In accordance with CFR 29 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Town & Country Underground, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 19263652(a)(1) which was contained in inspection number 314420308, citation number 01, item number 03, and was affirmed as a final order on 08/04/10, with respect to a workplace located at Harrison St. & S. Frontage Rd. Hillside, IL.    Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d)
