29 CFR 1926.304(h)(1):     Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:        On October 20, 2011 an employee was exposed to an amputation hazard while working with a cross cut saw that had the hood pinned in the up position.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.304(i)(1):     Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.    On October 20, 2011 an employee was using a table saw that did not have a guard covering the blade, exposing him to an amputation hazard.    In accordance with 29 CFR 1903.19(c), Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      On October 20, 2011 an employee was not provided with a safe means of access to the ladder jack scaffold he was working from, exposing him to the hazards associated with no safe access.    Abatement certification is required of this item in accordance with 29 CFR 1903.19(c)

29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    Instance 1.  On October 20, 2011 one employee was not protected from a fall while working from a ladder jack scaffold, exposing him to a fall hazard of 16.4 feet from the north east side of the property.    Instance 2.  On October 20, 2011 one employee was not protected from a fall while working from a ladder jack scaffold, exposing him to a fall hazard of 17.7 feet from the south east side of the property.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that the abatement is complete must be included with your certification.
