On or about January 25, 2012, in Bay #4, employees were exposed to 8-10 feet falls while climbing on top of the stored pipe in order to hook the pipe up to the crane.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 25, 2012, in Bay #4, employees were exposed to the ingoing nip points created by the metal wheels rolling against the metal rails used for the automatic ID grinder.
