29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than one inch (2.5 cm) wide:    a) On or about October 20, 2011, the employer did not ensure that the employees were protected from falls whlie working from the scaffolds where the scaffolds were not fully planked exposing more than 9 inches on the west ends of the scaffolds.

29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide:    a) On or about October 20, 2011, the employer did not ensure that employees were protected from scaffold collapse while working from two scaffolds where the scaffolds were in excess of 20 feet high from the ground, and not tied off to the building at a 4:1 ratio.
