29 CFR 1926.1404(a)(2): Where the assembly/disassembly is being performed by only one person, that person did not meet the criteria for both a competent person and a qualified person. For purposes of this standard, that person was considered the A/D director. Employees of a painting contractor were exposed to fall hazards while working from a man basket because the A/D director for the crane rental company did not meet the criteria for a competent person. The A/D director did not recognize that the anti two-blocking device was not connected.

29 CFR 1926.1431(d)(5)(v): When using equipment to hoist personel an anti two-block device that automatically prevents damage and load failure from contact between the load block, overhaul ball, or similar component, and the boom tip (or fixed upper block or similar component) was not used: Employees of a painting contractor were exposed to fall hazards while working from a man-basket hoisted by a crane because the operator from the crane rental company did not utilize an anti two-block device when hoisting personel.
