29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by:    On or about, September 20, 2011 an employee was attempting to remove a container off the top of the controls of the Formula 5070, when the machine started and pushed him under the controls. The employee suffered a thigh injury as a result of the incident. The rotating boards on the Formula 5070 and the Challenger screen-printing machines were not guarded where the employee(s) work to protect against hazards associated with rotating parts.     Abatement Certification and Abatement Documentation Required.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    The employer did not develop in the workplace, a written Hazard Communication Program for employees exposed to hazardous materials such as:     1) CDR - concentrate   2) SPSI - SC 1000 Fast Tack Adhesive  3) Solvex #8    All provisions of 29 CFR 10910.1200(f) through (h) must be covered in a Hazard Communication Program. Key elements include, but are not limited to the following:     1. Labeling,  2. Material Safety Data Sheets (MSDS)   3. Employee information and training.
