29 CFR 1910.22(a)(2):     The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition.Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided wherepracticable.    a) The employer failed to maintain floors as clean and dry as far as possible in the Dining/Kitchen Area and Nursing Areas (Assist Living, Health Services, etc.); in which resulted into multiiple recordable slip/fall injuries; three recordable injuries in  2008, two recordable injuries in 2009, three recordable injuries in 2010, and eight recordable injuries in 2011.

29 CFR 1910.1030(h)(5)(i):     The employer shall establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps. The information in the sharps injury log shall be recorded and maintained in such manner as to protect the confidentiality of the injured employee. The sharps injury log shall contain, at a minimum:    a) The employer had not established and/or maintained a sharps log for the recording of injuries involving contaminated sharps in which woulf describe the type and brand of the device, where the exposure incident occurred, and a description of the incident.

29 CFR 1904.29(b)(6): Employee(s) names were not removed from the 300 Log when employee(s) experienced privacy concern cases.      a) On or about May 20, 2011 an employee received a needlestick injury while discarding a needle into a sharps container and the case was not marked as a privacy case on the OSHA 300 Log for the calendar year of 2011.    b) On or about June 15, 2011 an employee received a needlestick injury while trying to pull the safety shield over the needle and the case was not marked as a privacy case on the OSHA 300 Log for the calendar year of 2011.    c) On or about August 12, 2011 an employee received a needlestick injury while discarding a needle into a sharps container and the case was not marked as a privacy case on the OSHA 300 Log for the calendar year of 2011.    d) On or about October 2, 2011 an employee received a needlestick injury while giving a resident an injection and the case was not marked as a privacy case on the OSHA 300 Log for the calendar year of 2011.

29 CFR 1910.1200(e)(1):     Employers shall develop, implement, and maintain at each workplace, a written hazard communicationprogram which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning,material safety data sheets, and employee information and training will be met, and which also includes the following:    a) A written Hazardous Communication Program had not been developed by the employer to educate employees on the physical and chemical hazards presented by the chemicals in the worplace such as but not limiited to Ecolabs' Wash n' Walk, Oasis 137 Surface Cleaner Degreaser, Oasis 122 Cleaner and Degreaser Concentrate, Oasis 146 Multi Quat Sanitizer, and Kook Klene.
