29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a)	Employer allowed its employees to work from working levels that were not completely planked.  Employees were exposed to falling 16 feet to the lower levels of the site.    **In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).**

29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    a)  Employer allowed its employees to access and egress a portable extension ladder that was not extended at least 3 feet above the upper level or secured to prevent the ladder from swaying or tipping.    **No abatement certification or documentation is required for this item.**

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:      a)  Employees were not provided a safe access to and from the working levels of the supported scaffolding system.  They   were exposed to falling 15.5 feet to the concrete ground below.    Walter Grzebieniak Building Maintenance, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 314463688, citation number 01, Item number 004 and was affirmed as a final order on July 1, 2010, with respect to a workplace located at 2206 Milwaukee Avenue in Chicago, Illinois 60647.    **In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.**

29 CFR 1926.451(g)(3)(i):     Verical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold and protected from sharp edges and abrasion.  Safe points of anchorage do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams or counterweights:      b)	The vertical lifelines of the employees personal fall arrest system were not protected from abrasion when draped over a masonry parapet wall.
