29 CFR 1926.303(c)(3): All other portable abrasive wheels used for external grinding, were not provided with safety guards (protection hoods) meeting the requirements of paragraph (c)(5) of this section:     (a) At the first level interior of the building on or about October 24, 2011, and at times prior thereto, there was no guard for the 4.5 inch diameter abrasive wheel on a 110 VAC Makita right angle grinder.  The grinder was used to sharpen tools.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    (a) At the north end of the building on or about October 24, 2011, and at times prior thereto, an interior stairway was not equipped with at least one handrail.  There were 15 risers from the bottom landing to a second landing and at least 6 risers from the second landing to the second level.  Each riser was 7 inches high.    (b) At the south end of the building on or about October 24, 2011, and at times prior thereto, an interior stairway was not equipped with at least one handrail.  There were 15 risers from the bottom landing to a second landing and at least 6 risers from the second landing to the second level. Each riser was 7 inches high.
