29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:    (a) In the rear hallway of the project on or about October 25, 2011, and at times prior thereto, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program for an extension cords that powered 110 VAC portable fans, battery chargers, and a Rigid cut off saw.  The extension cords were plugged into ordinary 110 VAC outlets; instead of ground fault circuit interrupter outlets.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. When metal covers are used, they were not grounded. In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or they did not have smooth, well-rounded surfaces on which the cords may bear:    (a) In the rear hallway of the project on or about October 25, 2011, and at times prior thereto, there was no cover on a 225 volt, three phase panel.  The panel was 4 inches deep, 20 inches wide, and 41 inches high.  The bottom of the panel was about 26 inches from the floor.  Employees carried metal studs, ladders and metal conduit in the area near the panel.

29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized:    (a) In the rear hallway of the project on or about October 25, 2011, and at times prior thereto, there was no tag on a 225 volt, three phase, main circuit breaker in an electrical panel.  The breaker was in the off position.  If the breaker was turned on, all the other breakers in the panel could be energized.  The other breakers in the panel supplied power to lighting, outlets and other circuits in the building.  Most of those circuit breakers were in the off position and needed to remain in the off position to ensure circuits being worked on were not energized.  The panel cover was off, the area was not secured to prevent entry, and there was no sign that prohibited entry.  Any person had the opportunity to inadvertently energize the circuits that needed to remain deenergized.

29 CFR 1926.451(w)(2); Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold is used in a stationary manner:    At the project on or about October 25, 2011, two of the four wheels were not locked on a mobile scaffold.  Employees were on the scaffold platform pulling wires through conduit.  The platform was 6 feet, 2 inches from the concrete floor.

29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination were not protected from accidental contact or breakage:    (a) At the project on or about October 25, 2011, and at times prior thereto, several 100 watt light bulbs, used for temporary illumination, were not protected from damage as there were no guards over the bulbs and they were about 6 feet, 7 inches to 7 feet, 1 inch from the floor.  Employees carried metal studs, ladders and metal conduit in the area near the unprotected bulbs.  The managing director reported that the light bulbs were installed the day prior to the inspection.
