An effective means of gaining a handhold shall be provided at or near the head of each vertical fixed ladder. Vehicles with a fuel system leak or any other safety defect shall not be operated. a) Onboard M/V SPAR SPICA, hatch number three: A dozer operator was trimming the hatch and with the door open. The door was re-enforced, designed to protect him from flying pieces of scrap metal entering the cab.
