29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary: a) On or about October 25, 2011, employees were not protected from injury while working on Alum-A-Pole pump jack scaffolds that were not secured as follows: 1. Bracing was not installed at the bottoms of all scaffold poles. 2. Bracing was not installed at intermediate locations on the scaffold poles to secure them to the structure.
