On or about September 12, 2011, at the above addressed jobsite, an employee conducting sewer installation in a trench five (5') feet seven (7") inches in depth did not have a safe means of egress.

On or about September 12, 2011, at the above addressed jobsite, an employee was  installing a sewer line in a trench measured at five (5') feet seven (7") inches in depth.  The trench was not provided with cave in protection, thereby exposing the employee to cave in hazards.
