29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) In the Waffle Room, workers were exposed to a trip hazard due to a ~ 3 inch diameter flexible hose which ran from a pump to the mixing drum. This flexible hose was approximately 6 to 8 inches above floor level. Workers had to step over this hose, and a worker was observed tripping over this hose.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: (a) In the Waffle Room, the waffle oven operator was exposed to the hazard of rotating parts, which occurred where the returning conveyor belt spooled onto the rotating drum which picked up freshly baked waffles. The operator of this equipment was periodically positioned between the north and south waffle oven lines, and this condition thus existed on both sides of the worker performing this operation.
