29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) At Apra Heights, building 4175, room 104, the ground pin was missing on a 110 VAC extension cord. Employees used the extension cord in conjuction with a Makita drill to drill holes in the concrete walls. Employees were exposed to the hazards of contacting electricity.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) At Apra Heights, building 4175, room 104, the female end of an 110 VAC extension cord was not secured to it's outside insulation. The cord was used to power a 110 VAC Makita drill. Employees used the extension cord in conjuction with a Makita drill to drill holes in the concrete walls. Employees were exposed to the hazards of contacting electricity.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: (a) At Apra Heights, building 4175, room 108, there was no cover plate for the 110 VAC light switch. Employees who turned the switch to the on or off position were exposed to the hazard of contacting electricity.
